Currently we have 4 caregivers with openings for patients.  If you are interested in becoming a patient please fill in the form below.  The information you provide will be stored by the operator of Maine Caregiver (Fred Kessler) who is also the webmaster and the only one who will ever see your name and information besides the caregiver that contacts you.  The only information we need is an email address to contact you with, any other information you provide will aid us in setting up Maine Caregiver but is optional.  If you are interested in becoming a patient please let us know and we will contact you soon.
